Add Palette and Edit Palette dialogs

Adding a palette works, but it is not shown in the Edit Palette dialog or in the
Palettes list until Krita has been restarted.

4-
Modifying a default palette will yield a save error (write protection) without
passibility to save a copy.

5-
Oh, and deleting user-made palettes would be a nice feature as well :)
