Okay, here's the patch. Looking at the mails it produces, it should be OK.
I haven't an Outlook/Exchange combination available, though, so maybe
Thorsten or Michael could check whether it works in real life?

To enable this patch: add

[Groupware]
ExchangeCompatibleAttachments=true

to your ~/.kde/share/config/kmailrc

By that way, I thought for a moment the config dialog GUI was
auto-generated from kmail.kcfg, but apparently it isn't. Is this because
of the old and the new KDE config framework?
