Various problems/bugs with actions/toolbars in a KParts::MainWindow

I've seen similar problems in KolourPaint as well.

I'm don't know the answers to your other questions but I know the answer to 
this one:

Rename them from "cr-" to "hi-" and the icons will appear for any theme, 
including oxygen.
