The current state is pretty good actually.  Frederik explains what's needed
to test it here
http://labs.qt.nokia.com/2011/08/23/accessibility-on-linux/ it's
usable with orca (once you enable at-spi2 and disable at-spi which I think
ubuntu still defaults to.)  I just set up my machine new last weekend and am
getting a test setup working here too.
