I think that's the wrong question to start with. 

First question would be what kind of tags you want to add, and how many 
different tags you would have. Then, is there a logical way to group those in 
a hierarchy... Like has been said, if you end up with too many different tags, 
a flat list becomes unworkable. Then again, if you only have a few tens of 
tags, a tree is overkill...

If you see that you end up with too many tags for a flat list, you almost 
automatically go to a hierarchy (one or more trees with a logical grouping of 
the tags). 

But even if you create a tag tree within digikam, you can still just apply the 
"leaf" tags, if you don't want hierarchical tags in your image files. You can 
set up digikam to store just the applied leaf tag(s), without the parent tags.
Or you can store the full hierarchical tags...

Which is best for *you*, depends on your image collection and the way those 
tags are to be used (by you, or by others) and/or exchanged between programs.
