new user trying to import data from csv

The first line does contain field names, and I have now tried clicking
on each in turn and clicking on the associate field button. I also tried
doing this for just one field. in neither case is anything imported.

I tried to attach a screen-grab, but it is too big to be allowed here.
It shows the preview window with the titles of the columns.
