An example Form (only radio buttons relevant for this bug)

SUMMARY
A PDF Form with a Radio-Button Group as Choices does not behave correctly when
the form is filled out using okular.

There are serveral issues:
- When selecting a radio button, the other buttons in the group don't get
deselected.
- Re-Selecting the default chosen - or any other previously selected - after
selecting any other  option seems impossible (never works).
- The "form edit" and "normal mode" display state of the form is sometimes not
in sync/not the same state
- multiple elements are shown as "chosen" although only one should be choseable

Overall the radio button implementation seems broken.

The default presentation might even not be correct (this part might relate to 
Bug #519407 as the errorneous default display is kind of the same).

STEPS TO REPRODUCE
1. Open a PDF File with a choice menu with radio buttons (cf attached file)
2. Choose any option
3. Choose any other option
4. Choose any of the options prior selected options

OBSERVED RESULT
1. Default display works in some forms - not in others. E.g. sometimes "all"
options are shown as "chosen"
2. Selecting in form edit mode does not show in "normal mode"
3. Re-selected a prior choosen option does not work

EXPECTED RESULT
1. Only Default/chosen Option is always displayed as chosen, others are NOT
displayed as selected
2. Same option is shown selected in both form edit and normal mode
3. One can selected any option back and forth and it works.

(cf. firefox internal pdf viewer, there everything works as expected.)
