Alternative MWE PDF Form (but this one is "working")

This alternative PDF Form shows, that the described behavior is not always the
case - in case of this document all form text field data is displayed as
expected.
This might help trace the cause of the issue.

(but since pdftk dump_data_fields dumps the Data of both forms "as expected" I
do believe this is an okular bug)
