Please at least just add some configuration option to change the default for
new documents from whatever is the compiled in default (fit width) to whatever
default I specify.

Almost every time I open a document in Okular, the first thing I do is
View->Fit Page.  I've been doing this for a year or three now.  It's a constant
pain, but I don't know any better way.  I have a wide monitor and "fit width"
will usually just show about the upper half of each page.
