I've often seen the question "Ok, you talk about PDF but what about the other formats?". For me this question is pretty simple. For file formats that can't store any kind of annotations (I read in this thread that dejavu is also capable of annotations) we can still use the present solution or go with this ".okular" format thing. Even though I think this is a waste of time since ".okular" will never be supported by anything else (most likely). 
I actually liked the idea of Tobias who said that if you put annotations in a file without annotation storage capabilities okular can warn you that these annotations will be stored in an external file.
To get a hold of these information there could be a button for these formats like "export annotations" or something like that.

BTW did something happen with "poppler can edit pdfs" or is there a plan to implement pdfedit into okular?
