It seems I'm already using the newest driver from NVidia (1.0.3123) and
I had just rebuilt qtvision from scratch before posting (make clean; make). 
I'm using KDE 3.0.4, XFree86 4.2.1 with Xvideo on Gentoo.   

By scaled video, do you mean that I can resize the TV screen any way I like
(I remember I couldn't do that as freely in kwintv)?  If so then yes, that works for me.
