This actually regressed further. Not sure which update (in Plasma? In my video
drivers? ...) caused this.
Currently, with Konsole 15.08.3-1, on dual screen as described above, a first
instance of Konsole will be unusable, not refreshing the content and just
showing a part of the desktop background.
A second instance will be usable, but the menu bar stays black.
Transparency behavior as described above didn't change.

Single screen (laptop screen only) still causes no problems at all.

I attached a screenshot of both the non-refreshing Konsole and the one with the
black menu bar. Very curious where to look for the cause of this problem.
