Just stumbled into the same bug.  Uptime on Konsole is about a week.

Konsole 2.8.3, KDE 4.8.4, Ubuntu 12.04

Help Menu - no problems
Settings - Configure Current Profile and Switch Profile are duplicated
Bookmarks - no problems
View Menu - Everything except split view and detach current tab is duplicated.
Edit Menu - Copy Input To is ok, everything else is duplicated
File Menu - New Window, New Tab, Quit are ok, everything else is duplicated

I'm hopeful this helps you see a pattern.

If I create a new window (ctrl-shift-n), the new window and menu are completely
normal.

If I detach a tab, the new window and menu are normal (this is an "easy"
workaround).
