The 'Here' in Open Browser Here refers to the current location of the program
running in Konsole.  It is visible whether or not you are right-clicking on a
URL.  The URL-specific actions are at the top of the menu ("Open Link", "Copy
Link Address")

I am open to suggestions for a less ambiguous way of naming the menu item to
make this clearer.

If the action doesn't open Dolphin, that may be a bug - although it depends on
your setup.  Can you try running:

xdg-open "file://$PWD"

And see if that opens the current directory in Dolphin?
