Do you see the same happening on recent transactions when importing twice with
KMyMoney 4.5? If not it is very likely that your institution decided to change
something in the way they present the downloaded data and the duplicate check
mechanism in KMyMoney gets fooled.

To avoid this, don't download all transactions but only those from the last
download.
