Just a newbie myself, but $KDEDIR/share/toolbar (/opt/kde/share/toolbar
on your machine) IS the kde_toolbardir.  If I understand, you have a
private toolbar for the app kisetl in which you want the icon
installed.  It seems to have ended up exactly where you told it too but
not where you wanted it.  Other apps in $KDEDIR/share/apps utilize their
own toolbar subdirectory if they need it.  Sounds like you need to add a
toolbar subdirectory to $KDEDIR/share/apps/kisetl and specify
$KDEDIR/share/apps/kisetl/toolbar for installation.
