how should document-renames be handled in 4.1 branch?

Sorry, I'm not familiar any more with the way this is supposed to work
in 4.1. If you want to fix it, you should try to do something in
DUChain::documentRenamed.

Just disabling the assertion might work for a moment, but will
probably lead to buggy behavior (check the highlighting in the
documents), and maybe even crashes, later. However, maybe this is the
way to go, it probably becomes at least better than before.
