[PATCH] new file-templates for Qt4 widgets

Hi,

this is for now probably the last patch I'm sending in to improve Qt4
support.

It does two things:

1. Adds templates for Qt4 QWidget, QDialog (plain+both "with Buttons"
version) and QMainWindow ui-files. It also selects the Qt4 widgets by
default when creating a new .ui-file through the qmake manager

2. remove's the "Open ui.h" entry from the context menu of .ui-files if
the project is a Qt4 project.

Regarding the last "todo-point", better qmake4 support, I'm not sure
wether I can do that because the qmake-stuff has much to do with parsing
and I'm not really good at understanding parsers ;-)
