List box widget for text and checkbox

Hi,

I'm using designer to develop a GUI for entering include directories
for a project management backend for kdevelop. So far I've not being
able to find the proper widget.
This widget should be a list box with two columns: a string column for
the directory name and a checkbox column for setting the directory to
be a relative path to the project.
Is it possible to create such widget with designer?
