Help with application templates conversion

Here's an example of a template I'm trying to port. This is the version 
correspondig to the new API, and the parts that are causing me trouble 
(actually they're not working at all) are indicated by #### blocks 
(there are 4 of them, in all).
Any help, is appreciated.
