Patch for bug #65644

Hello!

I've made a patch for bug nr #65644. You can now create new icons and remove
them correctly from the Makefile.am.

In /buildtools/autotools/addprefixdlg.cpp the QLineEdit is changed to a
KHistoryCombo to simpilfy the process to add new prefix. For the moment there
are six items there.

Could somebody review it?
