KMail (or rather the IMAP resource) has hardocded limit of two  connections per account. Single instance of the IMAP resource will never attempt to establish more than two connections to the server. 

What I suspect is that your email provider is tracking the number of sessions not based on the number of actually opened TCP connections, but rather on the number of LOGIN/LOGOUT commands issued. It is possible in some situation that Akonadi will not send the LOGOUT command before the TCP connection is terminated (for example due to connectivity issues, or because you simply suspend your computer and the connection times out) and so they think the connection is still active (which means you chose a rather poor replacement for gmail) - I'd bring this up with their tech support.

I've noticed that as well, but it only happens sometimes I think. Depending on the features your provider support, we might be hitting some less-used and potentially more buggy code path. 

I think there already is a bug report about this somewhere. 

You don't have to delete the cache though, just mark the emails as read in KMail too :)
