I loaded a live CD using KDE 4.1.3 and the menu options were exactly as 
described by Anne and Bruce. Thank you for your kind advice.

However, my development version of KDE 4.3 (using Kontact 4.3.0-154.1), does 
not have these options. I am sorry I did not make that clear right from the 
beginning.

There are features of the more recent versions that I really like, especially 
the aggregation mode in the message list view. It appears that when these 
features were introduced the view headers option was dropped from the menu, 
and now only appears as a non-persistent per-message option.

Because I am set on using the new features, but wanted to change the default 
header view, there really was no option but to edit the kmailrc file. This IS 
a fairly standard way of configuring linux apps, after-all.

So with the correct values, I now have best of both.
