Kmail filter.
The header was
To: "'xxx x xxxx'" <xxxxx xxxx com>, texhax tug org
The filter was
To  contains  texhax
The match failed: the logfile read
[15:48:10] 0 = "To" <contains> "texhax" ("xxx x xxxx" <xxxxx xxxx com>)
i.e. it seems the second address on the header line was ignored.   procmail would have matched it, IIRC.

[xxx replaces a real name, of a person I do not know, for privacy's sake.  I received the message as a member of the
list texhax tug org ]
