KAddressBook uses it natively.
There is a plugin for KOrganizer which wraps the calls to Akonadi in a way 
suitable for KOrganizer's access framework, but that has its own share of 
problems.

Native support for KOrganizer will hopefully be completed in time for KDE SC 
4.5
