Oddly, today I did a big bunch of work with KGrab and was waiting for the 
inevitable problem but it didn't happen. However, I do have 4.5.4 queued to 
apply, so there may be some hope.

I get icon glitchies all over - on the panel, hovering over panel items, 
the Google icon in the Konqueror toolbar, the clock hands. I've turned off 
all compositing and effects from way back.
