Does kde media manager perform fsck when its due?

Certainly it could be the reason. The automatic fsck is part of
system bootup which does only check devices in /etc/fstab. If your
external drive does not appear there when that bootup script is
called, it doesn't get checked.
The kde media manager doesn't act on the superblock counter.
