Have a look at the trash.desktop file. It should look something like this:
[Desktop Action EmptyTrash]
Exec=ktrash --empty
Name=Empty Trash
[Desktop Entry]
Actions=EmptyTrash
Comment=Contains removed files
EmptyIcon=trashcan_empty
Encoding=UTF-8
Icon=trashcan_full
Name=Trash
Type=Link
URL=trash:/
X-KDE-Priority=TopLevel
Apart from that, I can't think of anything else that would cause it to not
change the icon.
