The other menus are more or less coded into the application kdesktop.
Only the custom menu is created from files.

It might be possible to have level one submenus while still using the same 
fileformat.

If you think that would be nice, file a wishitem on bugs.kde.org, so that 
the respective developer knows about it and it doesn't get lost.
