You can still use this book as the changes where mostly internal.
On teh developer site http://developer.kde.org/ are two documents calles 
Qt3 portung guide and KDE3 proting guide describing the changes from Qt2 
to Qt3 and KDE2 to KDE3 respecively.

I think there is also a tutorial on writing IO slaves.

If any questions come up during development, please subscribe to the 
kde-devel mailintg list ans ask them there.

It has quite some traffic but you are very likely getting help from core 
developers who really know the stuff :)
