Yes, I'm pretty sure it can be done. But, you'd be better served to send your 
question to the kde-devel mailing list for the details :-)

QMenuBar inherits QMenuData which has lots of insertItem methods including 
ones for icons, pixmaps etc. Should be something there for you.
