Many thanks for the detailed log.

Could you get one more:

Run kdebugdialog and turn on debug for "ktp-auth-handler"

then run "/usr/kde4/libexec/ktp-auth-handler --debug --persist"
(note, that path may be different, adjust as appropriate)

Then disconnect and reconnect.
If that fails, please attach any output.
