I think pigale might be interesting for you:

Its editor is written in Qt.

There is also a much bigger lib, tulip:
  http://www.tulip-software.org/
but that might be overkill and it might actually be too big...

The best one could have been libqanava, but the project has just started and 
its author claims it should not yet be used for production software. Hm.
