  Actually, in KMail4 it's a bit hidden in the Configure KMail/Appearance/Layout tab, as "show message preview pane below vs. next to the folder list", possibly coupled to the long/short folder list view.
  
  I wouldn't be overly surprised if a significant number of users never get that far and thus never figure out what additional layouts can be configured (unless there's some sort of first-run wizard, I honestly can't remember that).
