We've already got such an On-Screen-Display both in Konversation and amaroK. 
In the latest amaroK version it works exactly like you suggest: when you 
change the volume, a scale is displayed in realtime. Looks nifty and is quite 
handy, as it provides a visual feedback.

At the moment muesli (inventor of the OSD stuff) is pondering getting a 
generalized OSD widget in KDELIBS for 3.3. This could be used by all 
applications that need such a display for whatever purpose.
