Here is a scheme is to split qt4 up inside /usr/local:
prefix ${PREFIX}
bindir ${PREFIX}/bin
libdir ${PREFIX}/lib/qt4
docdir ${PREFIX}/share/doc/qt4
headerdir ${PREFIX}/include/qt4
plugindir ${PREFIX}/lib/qt4/plugins
datadir ${PREFIX}/share/qt4
translationdir ${PREFIX}/share/qt4/translations
sysconfdir ${PREFIX}/etc/xdg
examplesdir ${PREFIX}/share/qt4/examples
demosdir ${PREFIX}/share/qt4/demos
This is very close to the current scheme, and so should have the least impact.
The biggest difference is that headers and libraries are given their own
directories. Binaries would have to remain suffixed with -qt4.
