 The problem is in non-kiosk mode, when  the user has the power to 
customize the toolbars. As soon as the version of the *ui.rc file is 
increased (because a new menu item is added, a new toolbar is added, a 
new toolbar item is added/removed), the user version of the *ui.rc file 
is discarded and you customization is lost.
