Status of konqueror / dolphin as file manager

As long as customisations are spurious, telling users to customise toolbars is 
intentionally misleading them. So let's stop that until kxmlgui is fixed.

Current options:
0. Do nothing.
	Pro: easy
	Con: misleading and buggy
1. Make inaccessible the edit toolbar dialog.
	Pro: We're not promising functionality we cannot deliver.
	Con: perceived as gnomeification
2. Fix it.
	Pro: the real way (tm)
	Con: not possible before 4.0, no decent design available on how to do it 
right
3. Workaround it.
	Pro: will keep settings
	Con: New buttons won't become visible on upgrades.

Next to complaining, let me present you a patch that implements (3) resorting 
to the "infinity" workaround from Ingo Klöcker and Alexander Neundorf and let 
me emphasise why the workaround is better than the current situation and why 
I'd like to have it preferred in KDE 4.0 over (0):

- The user changes the toolbar for a reason.
- Wiping customisations is from the point of the user clearly wrong and 
unexpected.
- Not seeing new toolbar buttons after upgrades can be dismissed as a minor 
problem as it is not a user-induced change and therefore will not be 
perceived as missing.
- The user can at any time explicitly revert to the current defaults (and thus 
make visible all new toolbar buttons for all upgrades) via the edit toolbar 
dialog.

The major problem with the workaround is that new menue entries will also be 
suppressed. This is no actual problem as you cannot miss things you never 
knew about. Besides when the real fix rolls in (KDE 4.1 or 4.2) all 
accumulated (non-customised) changes become visible at once.

Please review.
