If you talk about the icons on those buttons (in LTR, next points to the right
and in RTL ro the left), you missed my point. I am talking about the order of
those buttons.
The icons issue has been fixed in so many places (Meni Livne fixed it in
Konqeuror 3.0 beta1, many hacks have been made by various people on
KStdAction, I fixed it in konversation, etc....). I am talking about the
order of the button group, for which you said (also mentioned by pinheiro ):

and you are right. However, no application uses your rule. I assume the reason
is lack of support from KDE for this issue (see
http://bugs.kde.org/show_bug.cgi?id=92090).
I am attaching here an image which shows how amarok looks on my system. Look
at the bottom right. The problem is not the icons, but the order of the
buttons.
BTW, another ugly example of this issue can be found in KWord, the align
left/center/justify/right group. To test just run "kword --reverse". I think I
fixed it a few years ago, if it does work as expected, just test one of the
demos found in Qt4 with -reverse, and see for yourself.
