the speach api is documented on the kdeaccessibilaty website 
you can issue a dbus call to get kttsd to speak from an application
that is if you are any good at programming ,, although if theres a huck on the 
event you want to speak you could write a small script that would make that 
call to kttsd ,, 

for kde applications there is support for  knotify events to be spoken 
and kttsd plugins for kate , and konvi that will speak content

thats for kde3.x  i have not had chance to test it on kde4 as yet
