Hi,

The first options tune the behavior at start up to scan items changes.

digiKam is also able to detect changes while running of course, but i
recommend to apply large change outside digiKam without the application
session open. If the right option is turned on in digiKAm behaviors
settings, at startup the database will be populated accordingly.

Else you always have the choice to force digiKam to scan items using
maintenance tool:
