Covers getting lost/overlapped

Hi,

For no apparent reason the covers in my collection either disappear or get 
replaced by others from the collection. When I try to restore them, usually 
it works well, but sometimes the covers are not removed or restored to the 
wrong one. It's pretty much annoying having to restore each cover by hand.

I know without any debug info it's much about useless, but perhaps it rings a 
bell in anyone. (And I can't reproduce it either.)
