STEPS TO REPRODUCE
1. click on Vault applet in systray, click on "Create a new Vault..." button
2.

OBSERVED RESULT
"Create a new Vault..." window has different icons in task manager and its
window decoration.

EXPECTED RESULT
"Create a new Vault..." window should only have one icon.
