I just retested it.
All of the occurrences mentioned (two screens active while on login screen,
laptop screen underneath monitor, graphical glitches) are 100% reproducible.
In addition, aside from the menu glitch, I found two others:

1. Maximized windows will be maximized only up to 1366x768
2. Lock and login screens will also be confined to 1366x768
