Same Problem.
Upgraded from 14.10 to 15.04 and using kde ppa backport.
Firefox eventually turned up.
Blank page icons appears to: Thunderbird, Synaptics, pyCharm.

Some suggests that this is a theme problem:

Some entries in the KDEs System Settings also appears without icons, which
might be related.
