Just to make sure: Does
  ls $KDEDIR/share/apps/umbrello/pics/cursor-{initial,end}_state.png
show you those two files?
In gdb, you could set a breakpoint at worktoolbar.cpp:305 and
step thru the loop to find out which exact pixmap file is causing
the problem.
