bug fix: constructor for association should have document in it, this
is needed at minimum so that it is properly added/removed from
objectlist in parent. Memory leak/bugs in code generation can
otherwise occur.

bug fix: classes/interfaces owned by a package where not properly
being initializaed in umldocument upon XMI load [umldoc.cpp]
