Summary: restructuring plugin directory layout

Initial Comment:
The current system of generating a  
menu-hierarchy of plugins is both inflexible and  
confusing. Rather than deriving the structure of  
the menu directory from the structure of the  
directories the plugins are found in, we should  
use a system consisting of:  
  
1) A set of xml-files in an arbitrary directory  
structure  
  
2) An xml-file listing each of the plugin-files  
under 1) and specifying where those plugins  
should be placed in the menu-hierarchy.  
  
Advantages:   
- We don't need one directory per plugin.  
- Placement in the menu does not rely on  
alphabtical ordering  
- Possibility to place plugins in main menus  
(e.g. the save/load plugins under "File")  
  
Things to keep in mind:  
 In order to allow third parties to easily distribute 
additional plugins, it will have to be possible to 
combine several of the menu-description files 
under 2)
