I don't recall KDE doing that in a very long time (with the bad
exception of redoing some tarballs right before release without notice).
However, for serious issues I think doing a new release is a good idea.

Yes, but Albert was suggesting to do a new release as 4.9.1a (notice
the final 'a'). Using 4.9.1a or 4.9.1.1 as the number is ok for us
(Gentoo). I do recall in the last discussion about release numbers
that not all distributions support all numbering schemes, though.
