We are definitely adding API to stable branches after .0 releases,
and I wish that we can keep doing that. It's always API needed for bugfixes of 
course, not for new features.

This has always worked that way in KDE (unlike e.g. Qt)

So yes, kdebase-4.5.4 can depend on kdelibs >= 4.5.4.

And I don't see how this prevents distros shipping stable updates.
As I said: it's API needed for bugfixing, all that is needed is proper package 
dependencies so that one cannot upgrade kdebase (or another KDE SC module) 
without upgrading kdelibs first to at least the same version.
